Generally (CQ WW has zero-point QSOs) speaking, you are awarded points for each QSO you make. Multipliers are awarded for diversity in your QSOs. The first time you contact a new state, province, country, zone, prefix, etc, you receive a multiplier in addition to the points for that contact.
The points and multipliers for each contest are defined by the sponsor. So, it's important to read the rules for the specific contest you're in. At the end of the contest, you total your points, total your multipliers, and multiply the totals to get your score.

It is very important to read all the rules of the contest. Some contests have "kicker", special stations that give you extra point or multipliers if you work them. You would never know of this is you joined the contest and intuitevely thought about points and multipliers.
