This is a common occurrence in Writelog when doing SO2R.

If you call a station on the second radio, WL "reserves" that number 
for the QSO. If you don't make that QSO and for some reason you go 
back to the run radio to work someone, the "reserved" number is skipped.

Later when you use the second radio again, a new number is reserved 
and the original reserved number is lost forever.

Like I said, it happens all the time.
